Important Information and News regarding

The Collegiate Futbol Academy at Austin Prep  

Thank you for taking the time to familiarize yourself with this information about our Academy/Clinic.

Daily Start & End Times

The Academy starts at 8:00am each day. We encourage all players to arrive at 7:15 am (no earlier) on Day 1.

Each day thereafter players should arrive no earlier than 7:30am.
Each day will promptly end at 12:00 noon.

What to Bring.

All players should bring the following:

  1. Soccer Ball with your name clearly marked on it.

  2. Shin-Guards

  3. Water Bottle.

  4. A light Snack. NOTE: Please do not bring any food or other items that contain peanuts or peanut related oils, extract, etc.

  5. Sunscreen

  6. A mouth guard is recommended but not required.

Lost and Found

We will do our best to gather all clothing, water bottles, backpacks, soccer balls, etc., left behind throughout the week and attempt to find out to whom they belong. The Futbol Academy at Austin Prep is not responsible for any lost or misplaced items during the week.

Player Conduct

The expectation of each and every player is they behave and conduct themselves with the highest level of sportsmanship, respect and kindness at all times. Everyone is expected to fully comply with each and every request, guideline and instruction from all staff. Being courteous, polite and respectful is our minimum standard each and every day. Inappropriate language, rude or disrespectful behavior, as solely determined by the Academy Coaching staff, will not be tolerated. The Collegiate Futbol Academy reserves the right to terminate a player's participation at any time from the camp for violating these expectations. There is a NO Refund policy should this occur.

MA Legal Postings

 The operator of each Clinic for children shall provide a program of activities and physical environment which shall meet the general recognized needs of the participants and shall in no respect be in conflict with their best interests nor a hazard to their health and safety. The operator shall release participants only to their parents or an individual designated in writing by the player's parent unless alternative arrangements are approved in writing by the board of health. The operator shall print on an promotional literature or brochures the following: This Clinic, only if applicable, may need to comply with regulations of the Massachusetts Department of Public Health and be licensed by the local Board of health. The operator shall inform parents at the time application forms are provided that the may request copies of back/ground checks/ health care and discipline policies as well as procedures for filing grievances, if so determined by the Commonwealth of Massachusetts..

Privacy Policy

The Collegiate Futbol Academy, LLC: PRIVACY POLICY

Our Privacy Pledge. The Collegiate Futbol Academy, LLC (hereinafter “The Academy”) respects the privacy of the users, registrants and participants who have accessed the Academy online registration services and sports camps sponsored and conducted by The Academy. We recognize the importance of maintaining the confidentiality of your personal financial information, if any has been received. This notice describes the privacy policy and practices followed by The Academy. This Privacy policy explains what types of personal information we collect, if any, and under what circumstances we may share it, if at all.

Registrant Information. We collect only relevant information about individuals that is needed to register participants utilizing the online registration services of The Academy and/or retain relevant information necessary to conduct the sports event the registrant may be attending. We may collect personal and medical about you (registrant information) which is “nonpublic”. We collect information about you from the following sources: Online Registration Information. We retain personal information we receive from you on the registrations for sports events utilizing the online registration services of The Academy. This includes information such as name, address, age, grade, date of birth, parents’ names and contact information, e-mail, phone, health history and medical information and waiver, release and authorization. The Academy does not collect social security numbers. In order to provide our camp online registration services to you, the information you provide does with filter through and/or is administered by third parties (I.e., WIX; Smart Waiver, etc.) providers. However, they have access to this information only to the extent necessary to establish your sports event registration.

Email Communication. We DO NOT SHARE or SELL emails from our camp participant to anyone or any organization.

Personal Information.  We will not sell or share your personal information with anyone including independent third party marketers offering their own products and services. We may share your personal information in response to a lawful request issued to a court, government agency, or regulatory authority, or as permitted by law.

Our Confidentiality and Security Safeguards. Our employees and contractors are permitted access to personal information that they may need solely to perform their jobs and to provide service to you. Our employees and contractors have access to such personal information as necessary to establish your account request or respond to your inquiries. All employees and contractors are required to respect your privacy. No one except our employees and contractors have access to the Academy computer system and records storage. We have established internal security controls, including physical, electronic and procedural safeguards to protect the information you provide us and the information we collect about you. We will continue to review our internal security controls to safeguard your registrant information as we employ new technology in the future.

Online Privacy Protections. At our website, www.collegiatefutbolacademy.com you may register for our Clinic and you may communicate with us via e-mail. To protect the information you provide us online, we use multiple levels of security. The application information we accept online relies on industry standard “Secure Sockets Layer” (SSL) encryption to secure your transaction information and communication. Generally, our e-mails are not secure. However, if we ask you to e-mail us information other than your name, address, e-mail address and phone number, it will be obtained using a secure (SSL encryption) e-mail form.

We use this information to better serve you.

Protecting Children’s Information Policy. Our online registration services are not designed for or directed toward children. We do not knowingly solicit or collect data from children and we do not knowingly market to children online. We recognize that protecting children’s identities and online privacy is important and that responsibility rests with us and with parents.

On-Line User Agreement

The Collegiate Futbol Academy, LLC
ON-LINE USER AGREEMENT
1. Your Acceptance Of Terms And Conditions. In consideration of your use of the clinic/academy
registration and event management services of The Collegiate Futbol Academy, LLC (hereinafter The Academy) you hereby agree that the services are provided to you under the terms and conditions of this User Agreement (“Agreement”), which may be updated by us from time to time without notice to you.
This User Agreement is in addition to and supplementing the Waiver, Release and Authorization Form/Agreement executed by Parents and/or Guardians of Academy/Clinic participants.
2. Description Of Service In this Agreement, the term “services” means the online registration
services provided by The Futbol Academy at Austin Prep, LLC . The services currently include providing users with access to information about the camp, plus the ability to register and
pay for the event online. In this Agreement, the term “registrant” means the person designated
to participate in the event. Registrant understands and agrees The Academy only provides
event information and registration services and has no control over the events or over the
registrants experience at an event.
3. Minimum Age; Parent or Guardian by using this service you certify that you are at least 18
years old. If the person who you are registering for an event is less than 18 years old, you certify
that you are the parent or legal guardian of the registrant.
4. Information About You: You agree to provide true, accurate, current and complete
information when such information is requested in connection with an event.
5. Privacy Policy: You authorize the information you provide The Academy and its third party administrators on the online registration is provided to The Collegiate Futbol Academy, LLC in conjunction with the specific event for which you are registering. The Academy will not share your information with any third party without your consent. You agree that the Academy may retain this information for its own records and for purposes of promoting other sports events in the future. Your information is subject to our Privacy Policy, which we encourage you to read on this website.
6. Release And Indemnity On behalf of yourself, the registrant, and the registrants parents or
guardians, you hereby release and discharge The Collegiate Futbol Academy, LLC and its directors, officers, agents, employees, coaches, owners, staff and volunteers from all claims made on behalf of yourself, the registrant, and the registrants parents or guardians (and the executors, heirs, administrators, and assigns of all such parties) and all claims, damages, judgments, and costs (including reasonable attorneys
fees) arising out of or relating to the registrants registration, participation in an event. Further, you agree to indemnify and hold The Collegiate Futbol Academy, LLC  and its directors, officers, agents, employees, harmless from all such claims, damages, judgments, and costs (including reasonable attorneys fees) made by any third party arising out of or relating to the registrant’s conduct or participation in the Academy/Clinic/Event.
7. Links This online registration service may provide links to other World Wide Web sites or
resources. Because The Collegiate Futbol Academy, LLC has no control over such sites and resources, you acknowledge and agree that The Collegiate Futbol Academy, LLC is not responsible for the availability of such external sites or resources, and does not endorse and is not responsible or liable for any
content, advertising, products, services, or other materials on or available from such sites or
resources. You further acknowledge and agree that The Collegiate Futbol Academy, LLC shall not be responsible or liable, directly or indirectly, for any damage or loss caused or alleged to be caused by or in connection with use of or
reliance on any such any content, advertising, products, services, or other materials.
8. Disclaimer Of Warranties YOU EXPRESSLY UNDERSTAND AND AGREE THAT YOUR
USE OF THE The Collegiate Academy, LLC ONLINE REGISTRATION SERVICE IS AT YOUR SOLE RISK. THE SERVICE IS PROVIDED ON AN “AS IS” AND “AS AVAILABLE” BASIS. The Collegiate Futbol Academy, LLC EXPRESSLY DISCLAIMS ALL WARRANTIES OF ANY KIND, WHETHER
EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO THE IMPLIED WARRANTIES
OF MERCHANTABILITY AND FITNESS FOR A PARTICULAR PURPOSE. YOU EXPRESSLY
UNDERSTAND AND AGREE NO ADVICE OR INFORMATION, WHETHER ORAL OR
WRITTEN, OBTAINED BY YOU FROM The Collegiate Futbol Academy, LLC OR THROUGH OR FROM THE SERVICE SHALL CREATE ANY WARRANTY NOT EXPRESSLY STATED IN THIS AGREEMENT.
9. Limitation of Liability. You expressly understand and agree that The Collegiate Futbol Academy, LLC shall not be liable for any direct, indirect, incidental, special, consequential, or exemplary damages
(even if The Collegiate Academy at, LLC has been advised of the possibility of such damages) resulting from: (a) the use or the inability to use the service; (b) statements or conduct of any third party; or (c)
any other matter relating to the service.
10. Exclusions And Limitations: SOME JURISDICTIONS DO NOT ALLOW THE EXCLUSION
OF CERTAIN WARRANTIES OR THE LIMITATION OR EXCLUSION OF LIABILITY FOR
INCIDENTAL OR CONSEQUENTIAL DAMAGES. ACCORDINGLY, SOME OF THE ABOVE
LIMITATIONS OF SECTIONS 9 AND 10 MAY NOT APPLY TO YOU.
11. Notice You understand and agree that we may notify you via either email or regular mail. We may also provide notice of changes to this Agreement or other matters by posting or sending notices to
you. However we expressly reserve the right to change this agreement without
notice by modifying it on the public website where it is posted.
12. General Information These terms and conditions of this User Agreement constitute the
entire agreement between you and  The Collegiate Futbol Academy, LLC and govern your use of the registration service, superseding any prior agreements between you and The Collegiate Futbol Academy, LLC if any. The terms and conditions and the relationship between you and The Collegiate Futbol Academy, LLC shall be governed by the laws of the Commonwealth of Massachusetts without regard to its conflict of law provisions. You and The Collegiate Futbol Academy at Austin Prep, LLC agree to submit to the personal and exclusive jurisdiction of the courts located in Essex County, Massachusetts. The failure of The Collegiate Futbol Academy, LLC to exercise or enforce any right or provision of the Terms and Conditions shall not constitute a waiver of such right or provision. If any provision of the Terms and Conditions is found by a court of competent jurisdiction to be invalid, the parties nevertheless agree that the court should endeavor to give effect to the parties’ intentions as reflected in the provision,
and the other provisions of the Terms and Conditions remain.

No Medical Insurance Provided

The Collegiate Futbol Academy, LLC does not provide medical insurance for any participant in the Academy, Clinic or Program whatsoever. By registering for the CFA, each participant and their families acknowledge and understand it is their sole responsibility to pay for any and all medical bills and/or medical insurance needed at all times. It is the parents/participants sole, free and voluntary choice to send their child or participant to the Academy if they have no Medical Insurance and the CFA, LLC offers nor direct or implied warranty or guarantee of any medical insurance whatsoever at any time.
